Pre-Entrance Health Requirements

All incoming students to Johns Hopkins University are required to meet certain health requirements prior to attending. Failure to comply with these health requirements prior to your arrival on campus will block you from adding or dropping classes, interrupt your orientation and may result in a $100 health form completion fee.

ATTENTION: Due to COVID-19 we are waiving the $100 health form completion fee for Summer and Fall 2020.

It is still very important that you complete your health requirements prior to arrival on campus for the safety of our community.

Upcoming Deadlines

Pre-entrance health requirements must be completed by the following dates:

Early Arrivals: May 30

Fall admission: July 15

Spring admission: January 15

Post-doctoral fellows: One month prior to the start of your appointment

Completing the Pre-Entrance Health Requirements

Please carefully follow these steps to complete the pre-entrance health requirements:

Step One

Complete and sign the pre-entrance health form

  • Download the pre-entrance health form (PDF)
  • Post-docs: Download the PDF Document: Post-Doctoral Health Form (PDF)
  • No physical examination required
  • Pre-entrance health form and supporting documents must be completed in English
  • To verify your immunizations, your pre-entrance health form must be signed by your health care provider
    • OR, in lieu of a provider’s signature, you may attach a copy of your official immunization record to your pre-entrance health form
  • Please do not submit any CDs or actual laboratory films
  • Make a copy of your completed pre-entrance health form for your records

Important: Do not submit the pre-entrance health form until you have completed step two.

Step Two

Register for the Pyramed Health WebPortal and complete the required online health forms

Once you have completed Step One, follow these instructions to register for the Health WebPortal and complete the five online health forms.

Upload Private Insurance Card
(To only be completed by domestic students who have waived out of the university’s insurance plan.)

  1. Log in to the Student Health WebPortal
  2. Click on the Document Upload tab
  3. Select type of document (insurance card), browse to choose the file, and then hit “save”.
  4. You will then see the message : “Upload Successful! Your document is now uploaded.”

Special instructions for post-doctoral fellows

As a post-doctoral fellow, you will first need your Hopkins ID and email address to register your account on the Pyramed Health WebPortal. To obtain your Hopkins ID, email Linda Zeigler ( the following information:

  • Full name (differentiate between first/given, middle and last/family name)
  • Date of birth
  • Signed copy of your appointment letter
  • Hopkins/JHED ID (if provided)

Step Three

Submit your signed pre-entrance health form

Once you have completed the five online health forms, submit your signed pre-entrance health form to the Student Health and Wellness Center using one of the accepted methods of submission:

  • Fax to 410-516-4784 (include a cover sheet with your full name and date of birth)
  • Email a scanned PDF attachment of your completed form to
    Please note that photographed images of health forms will NOT be accepted!
  • Mail your completed form to:

    • Note: We cannot guarantee the arrival of mailed forms

IMPORTANT: Submitting your signed pre-entrance health form to our office prior to completing the online health forms via the Pyramed Health WebPortal will significantly delay processing.
