Staff IT Onboarding and Equipment Request Form
Please complete this form to request a computer when a new employee is joining your department, either as a new position or replacing a previous employee — or when additional IT hardware is needed for your department.
Note: Computer purchases for desktops and laptops are funded by a divisional account and replaced based on the divisional lifecycle process. Hardware purchased outside of the standard cycle or in addition to existing equipment is funded by departmental funds.
Please contact Ian Murphy for any questions regarding recommendations for tablets or other IT devices, which are typically funded by the department, not by the division. Tablets will still need to be configured by our IT staff, which can be requested by calling 410-516-HELP (4357) or placing a Helpdesk request through the myJH portal.
